The required level is the item level divided by 10, from 1 to 70. Ancient: 10% of its drops from level 70 enemies and level 70 treasure chests (9.98% on Nightmare and Inferno, where the Black Mist roll comes first). An Ancient item costs twice the gold to upgrade and sells for twice the gold. Black Mist: 0.2% of its drops from level 70 enemies and level 70 treasure chests on Nightmare and Inferno. A Black Mist item requires level 70, is never Ancient and stays tradable until its mists are revealed.

Ogre
0.5% per cleared encounter with it, on all difficulties. Met in Spire Hill Crypt Lower stage 3 (level 19 on Normal).

Legendary drops
Drops from enemies of level 10 and higher and from treasure chests they drop, only for heroes of its classes.
Mystery Gloves
Sold for Soul Shards by Lady "Shadow", the Mystery Vendor in each town: one Gloves of random rarity for the current hero. Online characters can buy it too, at the price the game server sets.

Enemy special drops are rolled once for every cleared encounter the enemy is part of, however many of it the encounter holds, in the places listed here. On Nightmare and Inferno every enemy in them is level 70.

Drop chances are the game's rules for offline characters; the game server decides the loot of online characters.
